Exponential Health DPC+

Exponential Health DPC+ is a membership-based primary care system designed to give you easier access to care, more time with your provider, and better support navigating your health care experience. It strengthens the front end support through primary care, prevention, routine labs, care coordination, and guided access to additional services when needed.

• Access to low-cost medications, in-house pharmacy support, and a trusted specialist network with transparent pricing

Yes. Members can connect with their care team by phone, text, video, and in-person visits, depending on the situation and need.

Yes. DPC+ is built for much more than treating illness. You can use it for preventive care, annual physicals, wellness support, chronic condition management, lab testing, medication management, follow-up care, minor in-office procedures, and guidance when you are not sure what care you need next.

Your membership gives you a consistent place to turn for questions, concerns, follow-up, and everyday healthcare decisions, with a care team that knows you and can help you act earlier and more confidently.

Yes. Exponential Health can help manage many routine medications, refill eligible prescriptions, and guide you to lower-cost pharmacy options when available. When a medication requires specialist management or additional evaluation, we help coordinate available next step.

No. You do not need to make a formal PCP change to use Exponential Health. If you choose to maintain an existing PCP relationship, our team can coordinate relevant clinical information with that provider upon request. Exponential Health providers offer easy access, continuous connected support, and no fees for visits or communications.

Yes. Spouses and children can also be eligible to enroll, depending on the membership option selected.

No. DPC+ is not health insurance. It is a membership-based primary care model that works alongside your existing insurance coverage.

DPC+ improves the part of healthcare most people use first and most often: primary care, preventive care, routine labs, follow-up, and day-to-day clinical guidance. Your health insurance still plays an important role for major medical needs, while DPC+ makes everyday care easier to access and easier to use.

Your care is led by licensed Exponential Health clinicians, including physicians and physician assistants, with access to additional specialty support when needed.

No. All DPC+ visits are included in the membership, which means you do not pay a copay or visit fee when you come in for primary care. No surprise bills.

Some services outside primary care may involve additional cost, including certain imaging, specialty testing, specialist visits, hospitalization, surgery, and other major medical services. When those services are needed, we help guide you to the appropriate next step with greater clarity around access and pricing.

For non-emergency concerns, contact Exponential Health first. We can help assess the situation and guide the right next step. If you are experiencing a medical emergency, call 911 or go to the ER immediately.

We currently have two DPC locations in West Michigan: our Cascade office at 5005 Cascade Rd SE, Suite B, Grand Rapids, MI 49546, and our Holland office at 3299 N Wellness Dr, Holland, MI 49424.

Traditional primary care is often shaped by insurance billing, shorter visits, and limited access to care providers. DPC+ is built to reduce friction in everyday care and give members a more responsive, relationship-based experience when questions, concerns, or follow-up needs arise.

Concierge Medicine and DPC+ both offer more time and access to your provider without the frustration of insurance-based care. The key difference is that Concierge Medicine is a comprehensive, annual membership designed for full-spectrum, proactive health management, including executive-level physicals, advanced testing, and 24/7 access.
DPC+ is a monthly membership focused on unlimited, convenient primary care, same-day or next-day visits, direct messaging with your provider, and coordinated referrals, ideal for everyday care and prevention.

Memberships are month-to-month. You can cancel anytime with a 30-day notice.

Yes. We strongly recommend maintaining health insurance or major medical coverage for hospitalizations, surgeries, emergency care, and other high-cost services outside the scope of primary care.

Yes. HSA and FSA funds can be used for eligible Exponential Health expenses, including DPC membership and other qualifying services.

No. Our model is membership-based and independent of insurance billing, which allows us to keep costs transparent and care patient-focused.

When care needs extend beyond primary care, we help navigate you to next step options. That may include imaging, medications, pharmacy support, or referral to a specialist through a more transparent, cost-conscious care pathway.

Same-day or next-day appointments are a core part of the DPC+ model. Our goal is creating more opportunity for members to reach the care team, communicate directly, and be seen when care is needed.

Both. Your membership includes access to in-person and virtual care, depending on the need.

Your membership includes select core labs at no additional cost, including CBC, CMP, lipid testing, TSH, A1C, urine microalbumin, PSA, and urine pregnancy testing. Additional in-office testing and many commonly requested labs are also available at heavily discounted rates.

Care navigation is a core part of the DPC+ model. We help coordinate referrals, support next-step decisions, and make it easier to move through the healthcare system with more awareness and less friction.

We make enrollment simple and provide support through onboarding so you understand how to access care and begin using your membership well.

Yes. To maintain the highest quality of care and same-day access, we intentionally keep our patient panel smaller than traditional practices.
